Synopsis

Dr. Miranda Gray is a psychiatrist who wakes up in a mental hospital as a prisoner. Her colleague, Dr. Pete Graham, is assigned to take care of her, as she is suspected of having a mental disorder. Pete explains to Miranda that she is a suspect in the murder of her husband, who was brutally killed with an ax. However, Miranda cannot remember anything that happened on that fateful night. While in captivity, Miranda is haunted by a female ghost she saw before the murder. The ghost uses a knife to cut Miranda’s hand and writes the words “Not Alone” on her flesh.

Despite the strange occurrences and Miranda’s plea of innocence, the staff at the facility refuses to believe her, assuming that she caused the harm to herself. Miranda’s lawyer, Teddy Howard, tries to prepare her defense for the trial, while Pete works to uncover the truth. When Sheriff Bob Ryan, Douglas’s friend, is brought in to question Miranda, he becomes enraged and lashes out at her. After the incident, Miranda is tied up and placed in the same cell as Chloe Sava, a former patient of hers who was raped during her time at the facility. Later that night, Miranda’s cell suddenly opens, and she manages to escape but is later caught.

Miranda uncovers evidence that her husband had been a rapist and torturer of other women, including Chloe. After Miranda sees a video where her husband is shown torturing and killing one of his victims, the police arrive and arrest her. She explains to Sheriff Ryan that her husband was part of a larger conspiracy of killers, and that Rachel, the female ghost haunting her dreams, was actually the daughter of her boss, Dr. Phil Parsons, who was a member of the conspiracy. Miranda also reveals that there is another killer still at large. The movie ends with an open and unsettling conclusion, implying the continuation of the story.
